Required Documentation for Eligibility of Clinical Participation
- Physical Exam: Students will need a completed physical exam by their health care provider dated after June 30, 2026. This form needs to be given to TCD by October 1, 2026.
- Tuberculosis (TB) Test: Students must have a 2-step TB skin test OR equivalent quantiferon blood test performed by their healthcare provider. The 2-step TB skin test or quantiferon blood test must be completed between August 1st and October 1, 2026. A copy of test results must be submitted by October 1, 2026
- The 2-step test consists of two TB skin tests done a minimum of 7 days and maximum of 21 days apart. The process to obtain a 2 step TB test will take up to 14 days.
- The quantiferon blood test gives results in a single test.
- Urine Drug Screen: Students must have a urine drug screen test and provide results to TCD by October 1, 2026 .The drug test must be a 7 or more panel urine drug screen test. The test can be ordered by the students personal physician and completed at the office or medical lab. Or students may also visit the TCD website for directions on how to obtain this test at a contracted outside lab (Approx. $51 fee)
- Titers for: Hepatitis B Titer, Varicella – (Chicken Pox), MMR – Rubeola (Measles), Mumps and Rubella (German Measles) **This should be scheduled as early as possible in the event vaccine series are needed. A titer is a blood test that determines the levels of immunity a person has against specific illnesses. If a titer test returns with low immunity levels, the student will need to provide proof of vaccination along with the titer test results. Test results and proof of vaccination (if needed) are required to be submitted by October 1, 2026. Please note that the Hep B vaccine is a multi-dose series over several weeks/months.
- Tetanus/Diphtheria/Pertussis Vaccination (TDAP) Vaccination - Proof of having received the TDAP vaccine within the last 10 years is required to be submitted by October 1, 2026.
- Flu Vaccine: All students will be required to provide documentation of receiving a flu vaccine in the last 6 months **The flu vaccine is often not available until early fall at pharmacies and physician offices. Check with your provider about anticipated dates when the vaccine will be available. Documentation is required to be submitted by October 1, 2026.
- Proof of current vaccination MUST include the following: (1) Student name (2) Clinic name (3) Date administered (4) Lot Number
- Covid Vaccine: If determined as required by a clinical site, student will need to provide proof of completed COVID vaccination by October 1, 2026. All students are encouraged to have this vaccine even if not required for clinical.
- Criminal Background Check: Students must complete a criminal background check prior to the start of clinical. Students will be required to provide a copy of their Social Security Card (or Individual Tax Identification Number - ITIN), and valid ID (Illinois Driver’s License, Illinois State ID or Current Passport). Background checks will be completed at TCD during the school day.
